Abstract:
Endovascular thrombectomy is often use as a rescue therapy for refractory cerebral sinus venous thrombosis (CSVT) . 1,2 Its performance is hampered by the absence of tools and techniques designed for clot removal in the venous system. Speci fically, the catheters and stent-retrievers used were designed for the arterial system and are undersized for the large sinuses. 3 Contrary, Copernic balloon is frequently used in the cerebral sinuses for fistula embolization. 4 We present a case of cerebral sinus venous thrombosis in a patient following meningioma resection. Active anticoagulation was restricted due to the occurrence of an epidural hematoma. Conventional endovascular thrombectomy with standard aspiration catheter sand stent-retrievers technique was unsuccessful at removing clots from the venous system. A 12 -French Penumbra Lightning aspiration system was combined with Fogarty maneuvers with a Copernic balloon microcatheter to successfully recanalize the transverse sinus. Post -procedural CTV and MRI showed almost complete removal of all the thrombi.
